Please read the Sample Forms carefully prior to completion of the following:-
- SVP Membership Application Form
The Membership Application Form must be completed in Full using BLOCK CAPITALS.
(please state N/A if details are not applicable)
- Garda Vetting Form
The Garda Vetting Application Form must be completed in Full using BLOCK CAPITALS.
(please state N/A if details are not applicable)
- Copy of Photo I.D.
Your Application must be accompanied by one of the Following:
A photocopy of your current Passport
A photocopy of your current Driver's Licence
If you do not have one of these please contact us in the Regional Office for alternatives
- Copy of proof of address
For example a recent utility bill to your home address
Please send you application to the Regional Membership Officer (address below) or bring it to your Training Session and hand it to one of the Trainers.
